Weekly COVID-19 Testing Is Required for Employees Visiting NF/ALF/CBO Patients. (At this time, this State requirement does not apply to regular home visits.)

If you are visiting patients in a Skilled Nursing Facility (SNF), Assisted Living Facility (ALF), Nursing Home (NH) or community-based organization (CBO), you are required to take a COVID-19 test weekly or as requested by the facility. When you arrive at the facility, you may be required to show proof of a negative test result within 7 days of your visit.

Be sure to submit your COVID testing results to CERT by emailing them to [email protected] every week.

Testing:

Many facilities are conducting testing on-site, and VNSNY has made arrangements for the facility to bill us directly for any tests they perform. You should not be asked for any proof of insurance.

If you are not tested at the facility, you can be tested at a city or state-run testing site. There is no cost to you —or to VNSNY— for these tests.

If you use another provider, such as your doctor, City MD or urgent care clinic, you may be required to pay all, or a portion of the cost, such as a copay.

If you are charged for the test, VNSNY will reimburse you for any of these out-of-pocket work-related costs.

Reimbursement:

If you have incurred any costs for COVID testing related to visiting SNFs or ALFs, you can submit your documents for reimbursement by doing one of the following:

  1. Submit your documents by email to [email protected]. You can scan the form or email a picture with your phone.
  2. Fax the documents to Partners in Care Employee Health at 212-290-3082.
  3. Mail the documents through the regular mail to the attention of: Partners in Care Employee Health 220 East 42nd Street, Floor 2 New York, NY 10017

After we hear from you, we will let you know if there are any further actions you must take for reimbursement.
