VNSNY’s New CMO Takes Home Care to the Next Level

June 26, 2019–Aimed at Reducing ER and Hospital Visits for High-Risk Populations, VNSNY Launches Care Management Organization VNSNY has added a new business unit—a Care Management Organization (CMO) that will work with insurers to provide their high-risk members with enhanced care management services in a home-based setting. Following approval from the New York State Department of Health, the new unit officially launched on May 6th. “With this new CMO, we’re providing insurers what they need most—a cost-effective way of keeping their high-risk plan members out of the ER and the hospital,” says Rose Madden-Baer, VNSNY’s Senior Vice President for Population Health and Clinical Support Services. The CMO is currently contracted with VNSNY CHOICE Health Plans to manage members of its two integrated Medicare-Medicaid plans, CHOICE Total and CHOICE FIDA Complete. “We’re actively exploring arrangements with other integrated Medicare-Medicaid plans in our service area as well,” adds Rose. Meet This Year’s Liota and Marren Award Winners!

June 19, 2019–This year’s winners of the Liota Award to Promote Cultural Diversity in Nursing Leadership and the Marren Leadership Award to Promote Evidence-Based Practice & Quality Improvement were announced recently as part of VNSNY’s National Nurses Week celebration. The honorees were nominated by their VNSNY colleagues and selected by a committee of nursing leaders. The winner of the Liota Award is… Margarita Luna, a licensed practical nurse with VNSNY Hospice and Palliative Care in Staten Island. And the winner of the Marren Leadership Award is… Dawning Greenstreet, a nurse supervisor with the DSRIP/Health Home program, administered by VNSNY’s Community Mental Health Services (CMHS). May 29, 2019–Michael Bernstein oversees a wide range of corporate functions for VNSNY, including Patient/Member Experience. You’ve identified hospitality as a key to improving the experience of VNSNY’s patients and plan members. Can you explain this? Whenever we’re delivering care, technical proficiency is obviously essential—but there’s another important element as well, and that’s emotion. How does the patient or plan member feel about their encounter with us? Hospitality means making our patients and members feel that we’re on their side. It’s about eliciting positive emotions by being empathetic, asking what we can do for them, and then finding ways to say “yes” instead of “no.” Hospitality is traditionally associated with travel and entertainment. How hard will it be to extend this approach to health care? It’s already happening. VNSNY Walks to End the Epidemic: AIDS Walk New York 2019

May 22, 2019–On a lovely spring morning last Sunday, VNSNY’s CHOICE SelectHealth team joined thousands of other participants for AIDS Walk New York in Central Park. “The SelectHealth team enjoyed a strong turnout from our 282 official registrants,” reports Shilonda Rosby, Partnership Outreach Advocate for CHOICE SelectHealth. “We also raised over $4,000 to support local AIDS service organizations, more than doubling our goal of $2000.” To see our walkers in action, click on the slideshow below!

VNSNY Golf Outing Raises Record-Breaking Amount for Hospice Veterans Program

May 22, 2019–Last Monday, on yet another beautiful spring day, friends and supporters of VNSNY gathered at the Wykagyl Country Club in New Rochelle for the VNSNY 2019 Golf Classic. This year’s outing raised nearly $285,000 — a record-breaking amount!  The money will go to support VNSNY Hospice and Palliative Care’s specialized services for U.S. military veterans. When an Infection Can Kill: VNSNY Launches Sepsis Awareness Campaign

May 16, 2019–Every 20 seconds, someone in the U.S. is hospitalized for sepsis, a condition in which a patient’s infection spirals out of control, causing organ failure and even death. “Sepsis kills over a quarter-million Americans each year,” notes Joan Cassano, Vice President for Population Health and Care Management. “Yet these deaths are mostly preventable—because if sepsis is detected early, it can be cured with antibiotics.” In recent months, VNSNY has rolled out a new screening protocol designed to help staff members quickly identify and treat potential sepsis cases. As part of this protocol, all VNSNY clinicians, care managers and home health aides are routinely checking their patients, clients and plan members for these six signs of potential sepsis: Clammy or sweaty skin Fever, shivering or feeling very cold Confusion Rapid heart rate Shortness of breath Extreme pain or discomfort “If any of these signs are present,” adds Joan, “the next step

Five Key Take-Aways from the Latest VNSNY Pulse Survey!

May 15, 2019–Our thanks to everyone who completed the VNSNY April Pulse Survey! 60% of staff members sent in their responses—up from 58% for our last Pulse Survey in December. Here are five key take-aways from the April results: 1) The overall trend is positive. Compared to last December’s survey, there was an increase in positive responses for every one of the 11 items the survey asked about. 2) We’re responding to the changing healthcare landscape. The single most favorable response from staff came on survey item #11: “VNSNY is taking steps to respond to the changing world of health care.” 81% of April’s survey respondents agreed with this statement (up from 76% in December), while just 4% disagreed. 3) Some areas of communication showed visible improvement … 70% of respondents agreed that “communication is a two-way street” at VNSNY, while 67% agreed there is effective communication within their program or department, and 66% said
