VNSNY Care360° Solutions Now Has Its Own Dedicated Website!

VNSNY’s care management organization is entering 2020 with a bang: In addition to its new name—VNSNY Care360° Solutions—the business unit has a new, dedicated website.

The VNSNY Care360° Solutions website, which you can see by clicking here, was created to drive awareness of VNSNY’s newest business unit, which launched last May. “The website is aimed at a professional audience of health plans and physician groups, who are challenged with managing care for their vulnerable members and patients—those at high risk for rehospitalizations,” explains Amy Margolis, a Director in VNSNY’s Marketing Department. “It educates our prospective customers about how VNSNY Care360° Solutions can manage these very complex populations by providing enhanced care management services in a home-based setting, helping to keep members out of the ED and the hospital, and reducing overall costs of care.”

The new website will help support the growing interest in VNSNY Care360° Solutions, with its steadily increasing client base. “We now have over 500 VNSNY CHOICE Total members enrolled in our longitudinal program, which manages high-risk plan members for 18 months at a time,” reports Rose Madden-Baer, Senior Vice President for Population Health and Clinical Support Services and director of the new business unit. “We’ve also begun a new contract with the insurer Healthfirst to manage the post-discharge care for 1,500 of their plan members this year. So we’ve seen tremendous growth in just eight months!”

The easy-to-navigate website was developed with input from the VNSNY Care360° Solutions team and other key stakeholders, and draws on insights from targeted customer market research. Key elements include:

  • A “Model of Care” page that highlights VNSNY Care360° Solutions’ unique approach to care management, supported by an eye-catching set of custom graphics and icons.
  • A “Results” section that showcases VNSNY Care360° Solutions’ positive outcomes, with links to specific case examples of how individuals have benefited from the program.
  • An “Expert Insights” section spotlighting the thought leadership of the VNSNY Care360° Solutions team.
  • A prominent “Contact Us” button, encouraging prospective customers to reach out.

“Everything on the website is designed to deliver one clear message,” says Catherine Callaway, VNSNY’s Vice President of Marketing and Development. “The message is that VNSNY Care360° Solutions has the experience, care management expertise and demonstrated results to help health plans improve outcomes, enhance members’ experience of care, and reduce costs.”
